Medical Providers
Our medical care providers consist of four neurologists: Dr. David Bear, Dr. Daniel Ross, Dr. Henry Porter and
Dr. Keena Risola; and a certified Physician Assistant, Shelby Brinson PA-C. We provide sservices at Baptist
Hospital, Gulf Breeze Hospital and West Florida Hospital. Our office locations include Pensacola, Gulf Breeze
(Andrews Institute) and Navarre.
Appointments
Office hours are Monday through Thursday 8:30 am to 4:00 pm and Friday 8:30 am to 11:00 am.
If you are a new patient, we will need you to arrive 15 minutes prior to your appointment to fill out paperwork (or
print and complete the Forms listed at the bottom of this page.) You must present photo identification and a valid
insurance card at check-in.
Although we schedule individual appointments, it is difficult to adhere to an exact schedule when dealing with
medical issues. We apologize for any inconvenience and appreciate your understanding.
If you arrive late for your appointment, it may be necessary to reschedule to another time or date.
If you cannot keep your scheduled appointment, please contact the office 24 hours in advance. If you fail to keep
your appointment and do not cancel, there will be a $75 fee for office visits and $250 fee for procedures.
Financial Policy
If you do not provide us with your insurance card at the time of your visit, you will be considered a “self-pay” and
required to pay in full at the time services are rendered. Co-payments and any outstanding balances are required at
Check-In.
Compliance with the payment terms are required to continue to provide services to you. Our billing department is
available during regular business hours to discuss the specifics of your insurance coverage and set up payment
arrangements at extension 107.
If you are a member of a Managed Care Insurance plan or HMO, and you are coming in for an office visit, a referral
or prior authorization is required from your primary care physician. If we do not have the necessary authorization
prior to your visit, your appointment will be rescheduled.
Telephone
Our telephone number is 850-438-1136. Incoming calls to our other unlisted numbers cannot be transferred and will
not be properly directed to the after hours service. Press “111” to reach the appointment desk.
When you call and leave a message, it is imperative that you:

Provide your full name
•
Provide your date of birth
•
Briefly state the nature of your call
•
Leave a valid telephone number at which we may return your call
We do not treat or diagnose illness over the phone. Please make an appointment to see your provider or physician
assistant if you feel you need to make a change to your current medications or your symptoms have changed.
No-Show Fee Policy
Since we value your time and seek to run our practice as efficiently as possible, we have implemented a no-show
charge to hopefully reduce the number of patients who fail to show up for an appointment without calling us in
advance to cancel. Urging patients to cancel an appointment in advance rather than just not showing up will allow
other patients to fill the open appointment slot and to be seen in a more timely manner.
ECCND charges the following fees for any appointment missed and not cancelled with 24-hour prior notice:

$250
missed procedure
No further appointment will able to be scheduled until this fee has been paid in full.
